(1) The purpose of research is an evaluation of clinical and paraclinical factors effects for VR-therapy efficiency during acute ischemic stroke period. VR rehabilitation course using was provided to 80 patients with diagnosed ischemic stroke. We assessed the significance of neuropsychological, clinical, demographic indicators on effectiveness of cognitive and physical VR rehabilitation programs using following parameter scales: MoCA, BI, NIHSS, IQCODE, HADS, MFI-20, AES, BPAQ-24, additional scales for praxis evaluation, semantic aphasia, perception and executive function. (2) Results demonstrate different correlations between rehabilitation factors. Attention, perception, pre-stroke cognitive deficit, affective disorders, sex, age and lesion side were significant both for cognitive and for physical VR-based rehabilitation programs. Also, some other factors for VR program choosing were highlighted. (3) Conclusion: VR rehabilitation programs, based on mathematical modelling with clinical and paraclinical parameters, can provide fully controlled virtual experimental environment for cognitive and motor functions recovery and expand the recovery potential of the patient.
